To reduce the discussion to the level of pointing fingers would not be of any help. I would rather we focus on the principle, rather than individualize it. Of course many examples abound - not just in ICT. However, I truly don't see of what value it would be to name names.
 
As for the plan, if you are working on one, that's commendable as I believe it is the way to go.
